List all subjects you plan to teach. New York requires instruction in specific subjects based on grade level per 8 NYCRR 100.10. An Individualized Home Instruction Plan (IHIP) listing textbooks, syllabi, and plan of instruction for each subject must be submitted.
New York requires quarterly reports and an annual assessment. Grades 1-3 use an alternative form of evaluation. Grades 4-8 must take a standardized test every other year. Grades 9-12 must test annually.

Relevant Legal Language
Pursuant to 8 NYCRR Section 100.10, the parent or person in parental relation shall provide written notice of intention to educate a child at home to the superintendent of the school district of residence by July 1 of each school year, or within 14 days if establishing home instruction at another time. An Individualized Home Instruction Plan (IHIP) shall be submitted within four weeks of receipt of the district's response to the notice of intent.
